Raktheswari belongs to the blood-goddess stratum of the Theyyam tradition, distinct from the better-known Raktha Chamundi. While Raktha Chamundi emphasises the battlefield aspect, Raktheswari is understood as the cosmic sovereign of blood itself — the life-force that sustains creation. Her worship is rooted in the pre-Vedic understanding that blood is sacred, not polluting.

In the primordial age, when the first beings were created, they had no vitality — they were forms without life. The great Shakti drew a line across her own wrist, and from the wound flowed a river of luminous red essence. She poured this essence into every creature, and they stirred to life. She declared herself Raktheswari — the sovereign of blood — and pronounced that wherever her essence flowed, she would be present, and no force could destroy what she had animated.

Raktheswari's performance features deep crimson face painting covering the entire face and neck. The performer wears blood-red garments and carries a ritual sword and a brass vessel symbolising the primordial blood of creation. A key ritual moment involves the pouring of turmeric-reddened water from the vessel onto the earth. The drumming pattern is distinctively slow and heavy, mimicking a heartbeat, gradually accelerating to a climax.
